To participate in middle school athletics, the following items must be completed and on file with the athletic office:

1.  Athletic Registration Form | Completed and turned in to the athletic director.
2.  Sports Physical  | (completed by health care provider) valid for three years.
3.  Participation Fee | $30.00 unless on free/reduced lunch $15.00.
